Arrival TimesPlease arrive 15 minutes early to check in. This helps your visit start on time. If you arrive late, we may need to reschedule you. Changing Your AppointmentIf you need to cancel or reschedule, call us at least 24 hours ahead:(360) 676-6177 (you can leave a message after hours) Whatcom County has a high need for timely care. When you cancel early, that time can go to another patient who needs it. Missed appointments We know life happens. But if you miss several appointments without calling ahead, we may need to adjust how we schedule your future visits, so we can keep care accessible for everyone. If this happens, we'll talk with you directly about next steps.

Unity Care NW receives grants that help lower the cost of care for our patients, including the Sliding Fee Discount Program. To qualify for and maintain these grants, we're required to collect some information from every patient:Household income and sizeRace and ethnicityAgeHousing situationInsurance informationVeteran statusFor income verification, please bring one of the following:Pay stubs from the last 30 daysLast year's tax return (if self-employed)Unemployment pay stubs from the last 30 daysA current benefit letter (Social Security, VA, etc.)Proof of other income (alimony, retirement, investments, etc.)Your last two months of bank statementsNo income to report?
